Our Story
In 2014, a group of Trillium Cardiologists began discussing the state of affairs of cardiac care in the community and the need to raise the bar. Based on personal observations, experiences, and an envisioned future, One Heart Care was conceived with a single focus in mind – One Place, Total Care.
What did we observe?
That while we worked cohesively under one roof in the hospital caring for our patients during their times of greatest need, we were functioning largely as individuals and small groups outside of the hospital. Our collective expertise of specialists and subspecialists were not adequately connected to offer the best and most comprehensive care for patients before or after hospitalization.
We observed that our community was dotted with many cardiology clinics, each offering different but incomplete expertise, resources and diagnostic testing. Some of these offered some type of connection to the hospital and to advanced cardiac care (specialized testing, interventions, hospitalized care) but the vast majority did not. Our patients were coming and going from the hospital, seeing their care passed from one group to another, having their testing performed at multiple locations and waiting for consultations from subspecialists. This type of inefficiency had real consequences – on the quality of care and peace of mind for our patients – and on the ability of our cardiologists to offer their best care.
What did we envision then?
A state of the art centre dedicated to comprehensive, longitudinal care. A place where patients could be seen by the right specialist at the right time with the right testing – all available under one roof. A group who’s unique promise is to care for cardiac patients across the entire spectrum of cardiac health and disease – one place for your outpatient cardiac care, and one group if and when you need hospital-based care. A place where we could leverage the skills of many to create new and innovative ways to deliver cardiac care.
Today, One Heart Care is that place.
